1. I am a veteran. How do I receive a Hamilton CapTel phone at no cost?
Complete the certification/order form in its entirety – you can download it here or we can send you a hard copy. The form has two parts – one to be filled out by the Veteran who will be using the phone and one by an independent third party professional. Once the form is completely filled out, return it to us by mail, fax or email.

2. Does my hearing loss have to be service-connected to be eligible?
No. Your hearing loss may have occurred at any point in time for whatever cause.

3. Why do I have to provide my last four digits of my Social Security Number and my date of birth on the form?
The last four digits of the social security number and date of birth are required to complete the registration process for the CapTel phone activation. This information is held strictly confidential by Hamilton CapTel and will be provided only to the FCC, if and when required of all Captioned Telephone users.

4. Why do I need independent third-party certification? Who can be my independent third-party professional?
Independent third-party professionals who are qualified to evaluate an individual’s hearing loss, include: Physicians, Audiologists, Hearing Related Professional and Government/Veterans Programs.

5. Do I have to go through the VA hospital to obtain the certification?
You are free to use the VA, but you are not limited to the VA healthcare system. You may use whomever you choose as long as the individual qualifies as an independent third-party professional.

6. How long will the phone be available at no cost? Will this change?
Internet Protocol Captioned Telephone Service (IP CTS) is regulated and funded by the Federal Communications Commission (FCC). The FCC may change IP CTS regulations at any time with little notice. As a result, Hamilton CapTel program rules, guidelines and pricing may change. 7. Is independent third-party certification the only way to obtain a Hamilton CapTel 840i?
You must have independent third-party certification to obtain the phone at no cost; however, you may also choose to self-certify and purchase the captioned phone for $75. Hamilton CapTel phones can be purchased online or by calling 888-225-9211.

8. I printed a certification/order form for this phone a few months ago. Can I submit that one?
Unfortunately, the previous form is no longer valid. A couple of additional requirements have been implemented since it was released. Download the new form here.

9. Should my independent third-party professional have any Privacy Act concerns?
No. All personal information is signed for by you, the veteran. Additionally, there is a statement just below the signature that states you authorize the independent third-party professional to submit the form, and your information, for you.

10. Is my VA aware of the Heroes With Hearing Loss program or the Hamilton CapTel phone offer?
It’s possible. We’ve reached out to several VA Medical Centers to tell them about our program and the captioned phone offer. We have not reached all of them, though, so yours may not be aware. If they aren’t, feel free to point them to our website, www.HeroesWithHearingLoss.org. We can provide informational materials, including a demo phone and display stand, at no cost.
