Complete the certification/order form in its entirety – you can download it here or we can send you a hard copy. The form has two parts – one to be filled out by the Veteran who will be using the phone and one by an independent third party professional. Once the form is completely filled out, return it to us by mail, fax or email.
The last four digits of the social security number and date of birth are required to complete the registration process for the CapTel phone activation. This information is held strictly confidential by Hamilton CapTel and will be provided only to the FCC, if and when required of all Captioned Telephone users.
Internet Protocol Captioned Telephone Service (IP CTS) is regulated and funded by the Federal Communications Commission (FCC). The FCC may change IP CTS regulations at any time with little notice. As a result, Hamilton CapTel program rules, guidelines and pricing may change. You must have independent third-party certification to obtain the phone at no cost; however, you may also choose to self-certify and purchase the captioned phone for $75. Hamilton CapTel phones can be purchased online or by calling 888-225-9211.
No. All personal information is signed for by you, the veteran. Additionally, there is a statement just below the signature that states you authorize the independent third-party professional to submit the form, and your information, for you.
It’s possible. We’ve reached out to several VA Medical Centers to tell them about our program and the captioned phone offer. We have not reached all of them, though, so yours may not be aware. If they aren’t, feel free to point them to our website, www.HeroesWithHearingLoss.org. We can provide informational materials, including a demo phone and display stand, at no cost.
